How is that irrelevant? It's a very good reason why he was an idiot for joining StarFleet, since he was unable to satisfactory execute his duty at all because of it. He's lucky StarTrek isn't realistic, because they throw you in jail for those things in real militaries, instead of sending you to a psychiatrist.

As for his holodiction, that only cropped up when he was on the Ent-D after he had already been in Starfleet for years, up until that point his record noted satisfactory service (by SF standards anyway) and some seclusive tendencies, although as Riker pointed out there should have been some regs against what he was getting up to.

His punishment would depend on the definition of holo-addiction and his personal neuroses within the framework of Starfleet regulations. If its classified like drug use is these days, then he would be discharged at the least. However, it is possible that the TNG era represents the low point of military capability for Starfleet, much like the late 70's for the U.S. Army, where there may simply be so many bad apples that it is impossible to weed them out, and a person who is inconsistently valuable at his job may be relatively indispensable compared to the godawful replacement for him they are likely to receive.
It's also possible that there are loopholes in the regs regarding this, which prevent him from being removed easily. There's also the possibility that Starfleet has a 'taken over by vastly intelligent superbeings' rule on the books, where anyone who fits the criteria is kept around for an indefinite time period in a harmless position, to see if anything interesting is retained in the victim's mind. The possibility of high placed patronage can also not be discounted.
And you would be surprised at how difficult it can be to get rid of incompetents, even when they have absolutely no redeeming qualities.

Besides, those two never got freaked out and panicked because the transporter would last a few seconds longer.
Pulaski never used the transporter except when forced to and was quite clear in her opinion of it as was McCoy, they are just as much idiots as Reg was for joining SF.

Actually I don't remember the transporters going crazy very often. Usually they just can't be used.
If he really were rational in his fear, he wouldn't go within 50 miles of the E-D because of the warp core.
I'm sorry, but I could never find anything interesting about an unworldly, unsocial, pathetic geek; a walking catalogue of neuroses who in any real world would never get one foot aboard a starship on the grounds of emotional and psychological unreliability. Barclay was a one-note joke of a character who existed soley as a freak-show attraction.

It seems that the writers overcompensated for the lack of depth and flaws in the main characters, and dumped them all into Barcaly.
It's worse than that. Barclay was not only a stupid character based on a stupid idea, but he was also a personification of every negative geek stereotype known to man. Gee, a socially retarded but technically brilliant geek. What a fucking original character! Why didn't they just throw in the dumb blonde, the stupid jock, the shy but polite nerdy Asian guy (whoops, they did that in Voyager with Kim), the violent black guy (whoops, that was Worf), and the over-educated British guy (oops, that was Picard)?
Barclay was proof that the writers of Star Trek subscribe wholeheartedly to negative stereotypes about scientists and engineers. In fact, brilliant scientists on TV are usually head-cases, and TNG does precious little to buck this trend.
