I see it more as institutional inertia than incompetence - why make changes to a plant that is working fine? As noted, the power plants built a decade later held up better.

Also, part of the problem is not making plant upgrades mandatory. The changes were made mandatory in the US, but the US could not impose that on Japan. Why didn't the Japanese government mandate these changes? That's a political failure, not a technological one.
