In this case it was apparently the fault of the signal man, or whomever was in that kind of role anyway.

BBC writes signal controller, which at the end of the day, is a dispatcher. If it was a signal maintainer who fobbed the job and messed up the equipment, it would be a different story, but the person made the deliberate decision to turn off the signal to allow a train to move when it should not have.
