Bascially, McCain is saying an Arab can't be a nice family guy.
That certainly seemed to be the implication, weather intended or not.
Lots of people in the media and campaigns have pointed out how Obama is not a Muslim or Arab. Disturbingly few have ever pointed out how wrong it is that "Muslim" and "Arab" are even considered insults in US politics.

I SUPPORT A NATIONAL GENERAL STRIKE TO REMOVE TRUMP FROM OFFICE.
I don't think that McCain necessarily meant it that way. I mean, there are a lot of things to rip him for, but that's not one of them. It was an off-the-cuff response on his part, and I don't think he entirely thought that part through.

Just like he didn't think it through when he contradicted himself on the economy by touting the virtues of deregulation while simultaneously saying that the government needs to clamp down on Wall Street. Just like he didn't think it through when he baited the Russian bear after the Georgia invasion by publicly and vocally taking sides before even acquiring the facts. This is not a man who thinks things through in general, but I don't think it's an excuse; it's more of an accusation.

Well, yes, but he's expected to be ready and prepared for things like that. It seems like he was caught completely caught off-guard by that accusation (which is inexcusable in his own right; nearly all of his attack ads have been dedicated to fostering the perception that Obama is, indeed, a secret terrorist) and wanted to counteract it immediately.
I don't really expect him to have all angles thought through in an off-the-cuff reaction like that, and I think it is unfair to say that he's racist based upon his way of trying to silence this nut.
However, I think it is perfectly fair to say that in light of his refusal to stop his atrocious "Who is Barack Obama?" line of attack, he wasn't being sincere.
