A Double Agent question

There are some spoilers in the following, if you just stmbled in here.

I recently finished Splinter Cell: Double Agent for 360. However, there's something missing. Though the proper last mission is in the JBA compound and involves your showdown with Emile, I know that there is a mission after that one, involving Carson Moss and the bomb he took into New York.

The problem here is that I haven't done this epilogue mission, nor do I seem capable of getting to it in any way. Perhaps you can only get this mission by making certain choices during the game? Not shooting Lambert, for instance?

You have to save at least 2 of the 3 targets (Lambert, Hisham or the boat that you place a bomb on) and have your trust meter almost full on the NSA side to get the bonus mission.
