Form making software?

I need to make some forms which consist mainly of lines to generate tables to (hand) write in. mac software would be best but pc software would do. I know I could just make lines on a page with a ruller but I'd like to this nicely and I can't draw a straight line to save my life.
if it helps i have Office for mac and windows.

How complicated are these forms? A page or two? Several pages? Excel with a combination of Word should probably do the trick.

Nah, you can just draw the tables directly into Word. There's a table drawing toolbar you can open.

You can´t draw a straight line with a ruler? That´s quite sad, actually.

Shift while moving the mouse usually gets shape things straight in most programs I use.

not complicated at all. 1 page should do titles across colums at the top with rows. basically i just need 5 fields across and what ever fits down the page with reasonable space to write in, 20 -30 entries per page. I'll investigate this table drawing toolbar.
as for drawing a straight line with a ruler, well it has to be more than just straight, it has to be angled and spaced correctly.

Use a word processor's table feature to generate the layout, or use a spreadsheet. Or even PowerPoint.
