Perhaps it represents how he's engaged in a battle to overcome the effects of the Fear toxin, in other words, Batman risks having his own existence overwhelmed by Scarecrow. Or maybe they just thought it would be a cool effect.

I think it's a reflexive statement by Bat's mind in how they both employ fear to their own ends.
Setzer's explanation also makes sense.
