Question about military access on EU3 Divine Wind

I just started playing the game recently and one thing I do not quite understand yet is how military access works. In times of peace, you need military access to move your troops and dock your ships in foreign territory. In war time, you can usually move your troops across foreign territory without acquiring permission first. Why is this so? And lastly, sometimes when you are at war, you are able to move your troops across foreign territory but then suddenly you can't anymore. Why is this so?

If the guy you are moving troops through is a vassal of the guy you are at war with, you automatically get the ability to move troops through their territory.
Give fire to a man, and he will be warm for a day.
Set him on fire, and he will be warm for life.
