Okay, forms are fucking hellacious. I need to figure out how to do a form script for a form like the following:

this and this (the latter example in the first form there)

Could I get some sort of help on this? I keep trying the damned sample page that I can download at the first example, but the CGI keeps giving me a 403 Forbidden error, so I don't know if it works or what.

Pearl it's not my strong point.

Really anything that will work is all that's necessary, as long as it can go on a web site.
