As far as we know, the working title of Episode III has been...Episode III, because they really don't think much about the final titles for the films until a little before it's officially announced, which will be in August as it was with the last two. I personally plan on ignoring any title rumours until LucasFilm announces it.
Revenge of the Jedi is often cited as being the original working title for Episode VI, but that's not true. The title was always going to be Return of the Jedi, but in 1982 Lucasfilm leaked "Revenge" to help spot early bootleged merchandise.
Unfortunately, they forgot to inform 20th Century Fox or Kenner of their plans, who started to produce advertising material and figure cards with the "Revenge" title until they were soon informed by Lucasfilm of the error.

No don't get me wrong; it's a great title and I love it. Hmmm, there's a Phantom Menace. Now the Clones are Attacking. (Insert Ep3 title). Ok, now there's a New Hope. Oh no! The Empire is Striking Back. Luckily the Jedi have Returned.
Sounds stupid, but it makes sense to me.

I'm almost positive the fighting on lava thing is bullshit. In the most recent Insider issue it shows Anakin and Obi going at it with no sign of anything that could resemble or eventually resemble a pool of lava.

Uts and I don't have any problem with them, because they're in line with the earlier films and we understand what Lucas is aiming for.

That doesn't surprise me. IIRC, the only reference to their duel atop the volcano is in the novelization of ROTJ. The very same novelization that was ignored with regard to Obi-Wan's and Owen's fraternal relation to each other. Quite frankly, I don't like the idea of Anakin and Obi-Wan having a climactic duel at all. It seems clear to me, based on dialogue in ANH, that Vader simply left Obi-Wan and they hadn't seen each other again until decades later on the Death Star. I'd rather see a Darth Vader/Mace Windu duel anyway. That would better demonstrate Vader's prowess, IMO.
