Three times. Once at the Hoth escape, when the trio of SDs collided, again after the asteroid chase where the Avenger loses the Falocn when she attaches herself to the Avenger's hull, and at the Bespin escape, when the Falcon's hyperdrive just barely saves them from the Executor.

They succeded at Tatooine however, when the Devastor captures the Corellian Corvette Tantive IV.
