Granted that Palpatine's "nice" appearance was a sort of Force plastic surgery, if I read the various replies correctly, I think part of the OP's question still remains unanswered, in a manner.
Was the veneer destroyed by the damaging effects of the reflected lightning, or was Palpatine unable to maintain it due to the amount of juice he had to put out to hold back Windu's blade?
It 's practically an academic distinction I guess.. and the former (it got fried off) is more dramatic.

Or more like Mystery Option 3: He just greased all the Jedi including their baddest lightsaber jockey... why he doesn't need to maintain the illusion/disguise/scooby doo rubber face anymore and getting messed up gets him more sympathy.

Because as they stated canon either agrees

Option #1: It was always his face, and Palpatine the disguise.

Option #2: Sith Lightning is rather detrimental to the health thus fucked him over.

Another slightly different question then.

Does anyone know how Palpatine explains to the galaxy his method for suriviving an assassination attempt by 3 Jedi Masters? Palpatine does not reveal to the galaxy at large that he is a Sith Lord and it wouldnt fit with his "pity me I barely survived a Jedi murder attempt" he cannot say that he defeated them himself.

So is the general assumption that Anakin the "Chosen One" and only Jedi brave enough to stop his evil Jedi bastard brothers arrived in the nick of time and killed all of the masters himself?

Or is there no good info at all on this so we dont really know?

IIRC the "official" story is that Anakin Skywalker showed up and put down the jedi masters before they could finish palpatine off.
And this is why you don't watch anything produced by Ronald D. Moore after he had his brain surgically removed and replaced with a bag of elephant semen.-Gramzamber, on why Caprica sucks

Since Luke Skywalker didn't shrivel like a prune when he got lit up with Sith lightning, ditto for Anakin/Vader (took a brief hit from Dooku and a concentrated barrage from Palpatine), my vote is for the "his real appearance was revealed" option.

There's a difference between using all the power you have to keep a Jedi Master from stabbing you and toying with a padawan learner.

In the "Samurai Jack" Clone Wars we see Palpatine as Sidious with wrinkle face prior to the events of ROTS. We also see Ventress use the force to make herself look totally different. Count Dooku even flat out says "...you mask you presence well, however I am not looking for a spy." If nothing higher up in the Canon contradicts this then I guess that means it is in fact:

Option #1: It was always his face, and Palpatine the disguise.
