Lets suppose the Jedi council grabs hold of some kid with Force sensitivity and initiates training...but due to psychological or personality problems, the would-be padawan is found to be suceptable to the dark side, and a conclusion is to terminate further training.
What happens next?

The would-be Sith is terminated by a Gungan hit-squad.

If they don'tt become sumones padawan by thwe age of tweleve they get dumbed into the agricultrul corps. Dunno what happens if they are shown to have dark side tendensies. It usally isn't notcied untill they begin killing things.

There is the Jedi Knights themselves, which is highly selective, but there are also the agriculturalcorps, the healing corps, and a bunch of others outlined in the Power of the Jedi sourcebook. Kid would go there and apply their force abilities to helping the republic in other ways.
Incidently, the existance of those groups mean that in addition to killing 10,000 knights, the empire has to kill all those, which should well outnumber the knights proper. Course lacking combat training, they should be easy prey for stormies, but still.

Was there any choice of leaving or were they "volunteered" into one of these Corps?

Dangerous force sensitives where taken into custody of the Jedi and put into what i assume to be a detention block in the Jedi Temple. Though this is just an impression i got from Shatterpoint.

I would have to agree, the Jedi never even intended to kill Aura Sing. Their goal was always to bring her to justice.
Say does anyone no whether the Old Republic had a death penalty?

I don't think we ever get to know this exactly, but if we assume that the Old Republic not has
that much influence on member systems, (after all, it didn't have a united army but a lot of regional defence forces) then it's possible that it varies from sector to sector, like it varies from state to state in the USA.
