According to Bruce Timm, when they were in the early stages of pre-production for ROTJ, he was discussing the project with Paul Dini and Alan Burnett. They had wisely made the choice to not utilize the Joker's original re-design from TNBA and wanted to go with a re-design that was more in line with his BTAS apperance (although early drafts of the script would have had the flashback Joker in his TNBA design and then the future Joker in a re-design, explained away utilizing genetic manipulation when he implanted the microchip on Timm Drake).
Anyway, Timm just couldn't get the design he wanted and spent the meeting doodling on a pad with various Joker designs. Well, at that point in time, Thomas Harris' third novel in Lecter trilogy,
Hannibal had just been released. This was at the back of Timm's mind and soon, he began thinking more along the lines of a Hannibal Lecter-esque Joker -- and they got it. That's why the future Joker has the slicked-backed hair and red eyes (and why they asked Mark Hamill to tone down his performance).
Oh, and the influence for the hands and their long, stangler-esque fingers? Boris Karloff's Frankenstien; Just like
Family Guy's Seth MacFarlane is to Richard Harris, Bruce Timm is apparently a huge Boris Karloff fan and this was his way of paying tribute to one of his idols.
