One of the great ones of the last 5 years for me is the first Bioshock when your realize...
Spoiler
...that Ryan is not the villain after all and that 'Atlas' has been manipulating you for the entire game with the trigger words "Would you kindly".

+1s for Luke's father and the ending of Blake's 7.

Adric dying in Doctor Who was a massive one for me as well.

It was more the fact that he was such an irritating prick to begin with, then as soon as he became interesting they bumped him off.

It was also a really pointless death. At least Katerina got something vaguely worthwhile, but Adric? Staying behind on a crashing freighter, trying to save it from hitting a plent destined to be hit anyway. That's a bummer.

The first time the Dalek LEV-I-TATed up the stairs.
After decades of 'lol stairs!' that was both shocking and surprising and terribly amusing all at the same time.

Oh and of course, Spoiler
Wash's death in Serenity.

It wasn't pointless, the freighter was going to hit future earth to destroy the conference that would create a military alliance to eradicate the Cybermen, Adrics tampering with the flight computer put the freighter into a time warp that sent it back 65 million years and caused the Cretaceous–Paleogene extinction event.
Adric died not knowing that his efforts had not been in vain but it certainly wasn't a pointless death.
So I stare wistfully at the Lightning for a couple of minutes. Two missiles, sharply raked razor-thin wings, a huge, pregnant belly full of fuel, and the two screamingly powerful engines that once rammed it from a cold start to a thousand miles per hour in under a minute. Life would be so much easier if our adverseries could be dealt with by supersonic death on wings - but alas, Human resources aren't so easily defeated.

Imperial Battleship, halt the flow of time!

Interesting. My interpretation last time I saw Earthshock was that the freighter was already spiralling back in time (the Doctor says as much to the Cyber Leader to taunt him that he's failed. Lt. Scott and co eject int he escape pod when the freighter is already 65 million years in the past, so Adric staying behind was worthless.
