(edit: hell, they'd have to be to fit on that weapon system) And a high airburst will generate minimal amounts of fallout, by that time it should start dispersing. Frankly, the fallout plume is of minimal importance right now ... the big issue is all the people dying from poisoned water from the Black Rain, massive burn casualties and other nasty things.
If it was an airburst, there is going to be little fallout beyond the immediate area. I also think you were overstating some of the effects - remember that weapons effect scale roughly to the 1/3rd power in yield (since they're spreading in three directions). Now, the terrible infrastructure will screw them over, but even terrain effects can mitigate the losses. Look at Nagasaki, for example, which missed its optimal aim point.
