There's a disbelief factor. I once stood at my office window, noticed a guy urinating on a co-worker's car, and was so gobsmacked by the surrealness of it that after a moment all I thought to do was pick up the phone, punch his extension and say, "John, there's a guy out front urinating on your car."

For people not used to sudden emergencies, it takes time to figure out what to do. What is there to not understand? This is the reason why emergency service workers, soldiers and whatnot have to go through training.
