Do I really have to point out the usual: the US is not the world? 
It suddenly struck me that that tiny pea, pretty and blue, was the Earth. I put up my thumb and shut one eye, and my thumb blotted out the planet Earth. I didn't feel like a giant. I felt very, very small.

Yeah. We really often take for granted how fucking amazing modern technology and medical science really is. Average life-expectancy (from around 20 years in the Neolithic to around 70 years today) is another incredible indicator of how far we've really come. Does a diptheria vaccine work less well in other parts of the world than in the US? Saying "the world" implies it's the same all over as we know it's not - plenty of third world countries have appallingly low vaccination rates and several of those diseases are still endemic in those places. Which is a damn shame because there's no reason for that suffering to still be occurring other than apathy, disinterest, and in a few cases, active campaigns against vaccination.

People are still dieing from diptheria in Africa. Vaccination rates matter a lot more than just effectiveness. As a side note, while it does not involve vaccines, we are also on the verge of eliminating dracunculus, which would be the first parasitic disease rendered extinct.
