When Garands go bad.
Either the 14th or 16th round (can't remember) through a brand new M1.
I was loading a single round, new Winchester ammo, and let the slide release forward slowly. The weapon was on safe and there was no finger near the trigger.
I believe that it was among the 10 thousand (10,000) in the latest run.
The weapon was returned to Springfield this week, and is in their possession.
