Well I don't.

But what about the other twenty million d00fs on the Internet who use nothing but Outlook Express and Idiot Exploiter (unpatched, no less!) and run with no firewall of any kind whatsoever?

This spam proxy doesn't care if you use Outlook or not - only that you execute the file. It's a classic trojan. It indeed will catch people without an outbound firewall as well.
