It's a country of dirt poor brainwashed people. If the brainwashing and indoctrination doesn't keep them from rebelling, the constant struggle to just survive will keep it off their minds. You guys are missing the point. Yes, the NK people are downtroden and brainwashed, but what about the military and political elite? How hard would it be to kill him, announce that he died of natural causes (heart attack/stroke thake you pick) and another one of them rise up to assume his position?

KJI is very picky about choosing who gets to be his inner circle, and those that do are the ones who Kim feels are the least likely to try and overthrow him. Besides, none of these higher-ups could hope to depose KJI without support from at least part of the North Korean military, which happens to have it's ranks filled with grunts that are brainwashed to follow the Great Leader's every whim. That, coupled with KJI's paranoid security measures and secret police, and it would be exceptionally difficult for one or some of them to plot against him without being discovered.

Or someone with a very strong ideological belief that he believes is worth dying for (or will repay him in another way). Islamofundies come to mind as an example of such.

The only way for it to stop if for someone really stupide to come into power at the top, or for an outside force to act. Historically, I'd say that the vast majority of regimes like this would have been easily conquered by their neighbors. Recently wars of conquest seem to have lost their polarity, and though this has huge benifits, it has its few disadvantages as well.
Its possible, if you're willing to sacrifice enough, to prevent coups with almost complete certainty.
