The joined Trill have a symbiont inside them that has all their memories.

Ezri Dax has all of Jadzia's memories, but not necessarily the same personality.

Not only that, but more importantly, the Trill have a LAW that states that they cannot go back with the same marriage partners, so that the symbiont can experience more, and breaking this law means that the symbiont will not go to a new host and will die when the old host dies (this was all in the episode where Jadzia wanted to go with another [female] Trill who was formerly her wife when she was a male...rather nasty episode, with Jadzia making out with another female Trill...yeech).

So Ezri Dax *couldn't* go back with Worf.

Since it is going to happen, it´s a little too late for that. All we can do is hope for the best.
