Red Koopa Shell? What about the Blue Koopa Shell? It has 100% accuracy- I've only managed to dodge it once, by an incredible stroke of luck I could never replicate, and always strikes the person in first place and takes out them and anyone nearby.
Also, I think Fire Flower and Screw Attack count as power-ups.

The Mega Mushroom from New Super Mario Bros! The final bosses- I walked over the first and jumped on the second and won.
Given my limited game playing here's some of my favourites.
Chainsaw (Doom) - It's a chainsaw and it rocks. It's iconic of the Doom games and is generally a hell of a lot of fun.
Lightsabre (Jedi Knight) - A melee weapons that does more than kill it's opponents, first one I encountered in an FPS.
Plasma Grenades (Halo) - A hilarious grenade that makes multiplayer that little bit more entertaining.
Shock Rifle/ASMD (Unreal) - A truly challenging weapon to use, nailing someone with a well-timed (and aimed) shock combo is one of the most satisfying things in an FPS.

RYNO -- Fires nine independently-tracking missiles, each at least as powerful as the Devestator(regular missile launcher)
RYNO II -- Fires auto-homing, indepently-targeting missiles at a rate of three to four a second.
RY3NO/Rynocirator -- Starts off similar to the original RYNO, but after the first upgrade or so, the missiles become disentergration beams that can usually kill an enemy even if they survive the initial power of the hit. The final upgrade is the Rynocirator, where the beams, instead of seeking enemies, converge on a single point and detonate in a flash of energy that burns everything but you to ash, literally.
Of course, damn near all the weapons in the series could count as my favorites. At least 65% of them would be unbalancing in most FPS multiplayer games, especially when properly upgraded. Ratchet: Deadlocked is one of the few games I've seen where the sniper rifle is considered as powerful a close-range weapon as it is at a distance.

Ohhh yeah, the fusion rifle and its variants is always one of my favorite weapons. It's followed closely by the Suck Cannon and the morph-weapon-du-jour.
Incidentally, for anyone who hasn't played the games, "RYNO" is an acronym for "Rip You a New One". The RY3NO, therefore, is "Rip You Three New Ones", something I always thought was amusing.

Able to spit out ~100 rounds per second, and the smallest burst you could fire was ~25 rounds. A single tap was all that was required to turn any enemy in the game into a shower of blood and limbs. Add to that you could carry up to 80,000 rounds, which was enough for 13 minutes of sustained earsplitting fire

The Land Shark gun from Armed and Dangerous was brilliant. It was a good area weapon, with a dash of humor sprinkled in. There was another great weapon on that game, the anti gravity bomb- basically a giant corkscrew that flipped the world upside down. You, the character, would dangle onto the handles while all the enemies would fall off the planet. When it switched off, then all the enemies would fall to their doom.
So yeah, put those two in there. And the fact that the Jedi Knight series lightsaber is not on the list is a tragedy of epic proportions.
I'd also like to suggest a weapon of skill. The piano wire from Hitman series. While not as gory or visualy awesome as other weapon, sneaking up on some poor fucker and watching 47 choke the shit out of him has always been a good stress reliever for me.

Sorry about the OT Necromancy, but for some reason, the Arch-Vile was a lot weaker while it was resurrecting something. Or maybe it was just that it had to stand still for a few seconds while you shoot it. Anyway, that's how I dealt with them. Pain elementals pissed me off, though; Try to shoot them, and it spawns out a Lost Soul to take the rocket, like a good little meatshield.

Spinfusor was just another rocket launcher with a new model and you know it.
