Pharma is probably the most scientifically based portion of medicine anywhere, but it's only a small part of medicine overall. Who is investigating the efficacy of common surgical procedures? Who is investigating the accuracy and specificity of various tests? What is the best management for the various manifestations of heart disease? Diabetes? What is the best way to treat severe extensive burns? What is the best way to avoid premature birth, and to deal with premature babies?

Outside of pharma, you'd be surprised how much medicine is based on tradition rather than hard science. Not that those traditions are all bad - but very few of them have been proven effective.

How?

I thought we were talking about "conventional medicine", not "big pharma exclusively"
