Instead, they must face an army of Deep Ones, Star Spawn, and Shoggoths, ten thousand strong.
The forces defending Helm's Deep are the same as in the book/movie.

The moment that they see those *things* come into view every defender goes completely insane. Half of them would probably kill themselves.
Possibly a few of the names characters may not succumb immediately, but...
Lets see...
Assuming that its ONLY deep ones. Sanity loss for seeing a SINGLE deep one is 0/1d6. So if the characters make a sanity save, they lose 0 SAN (out of a possible 100, though NO character in my experience has ever had a SAN of 100, and after the Balrog etc etc etc the FotR certainly wouldn't). Assume then that every sanity roll is a 1. They would have to make 9,901 successful sanity rolls not to go permanently insane. Thats a 99% success rate thats required.
If we are to put in the Shoggoths and Star Spawn (both have SAN loss of 1d6/1d20) then they all go insane as even if they make their sanity saves, they still lose a minimum of 1 sanity point.
Helms Deep falls as the entire defensive complement breaks down to gibbering lunatics.
