Cannon Fodder had some INSANELY hard levels. Though, a friend and I played it enough that generally we could get close to the final levels (if not actually complete them) with the original squad (ie NO casualties).
And another REALLY hard level:
The theme park (Ghost Dance? - running from memory here) on Rainbow 6... Trying to get close enough through the boat ride canals to spring the hostages without getting rumbled by tangos... Nasty.

Panzer General. There just wasn't a suitable difficulty level at all. On easy settings, the computer is basically totally gimped, and just sits on the objectives waiting for you. Turn the difficulty up, and the computer still just sits on the objectives, except now its units just mysteriously fight better. Regular infantry with zero experience beating back my 4-star panzer unit in open ground... yeah, right.

Funnily enough, I typically get pissed off with really hard games and stop playing, but in Ninja Gaiden every death gets the "I screwed up, let's try again" response, instead of "this is total bullshit, I'll play something else". That game has precisely the right difficulty level for me.

Devil May Cry 3. I blitzed DMC1 but i take no shame in saying that before i got Agni and Ruda i had to take the easy option that the game offered me. First and so far only game that i have had to complete on Easy difficulty first.
The defend the farm mission in the first part of Mechcommander. I finally managed to beat it once i learned of the miracle that is the minelayer, combined with the mad cat luckily salvaged from the earlier mission.

You think Ninja Gaiden is hard? Try Ninja Gaiden Black with its Super Insane Ultra Badass Extreme Ninja Grandmaster of the Universe mode, which is so hard that if you die, a real ninja comes into the room and kills you.
On that ninja related note, the Flash game N can be fiendishly difficult, even though it is extremely simple in practice. You need reflexes measured in the microseconds to be able to survive some levels for a few moments, let alone complete them.

Unfortunately no, it's a different world. It also uses a different system, a generational storyline (which by the way is quite poorly implemented).
I highly recommend the PS2 Romancing Saga remake though, it's a beautiful game.

X-Wing did. I think they killed the mission time limit in the Collectors CD version. The original FDD version certainly used to boot you if you ran out of time. Not that it was ever a real worry.

Both games had a couple of missions that were utter sods. There was an escort mission somewhere in the third campaign of TIE Fighter that gave me real grief, at least it did playing on Hard, whereas the rest of the game wasn't all that much harder. Wave after wave of torpedo launching craft, that really did have it in for the ship you were guarding.

I never managed to finish the last mission on X-Wing Alliance, until I cheated like hell. I was much younger, so perhaps I just need to go back and replay it, but even with god mode on, I'd always be encompassed by the fireball until I got lucky. I got hellishly confused by all the tunnels and things.
Homeworld 2 was impossible at mission 3 or so, but then I just made the enemy fleet that appears in front of you and obliterates you come to me instead, for some reason making it a lot easier. After that, HW2 was actually pretty straightforward aside from one mission where victory lay in realising that the Vaygr battleships concentrate virtually all their firepower forward in a limited arc - didn't notice that until I'd had a lot of vs CPU maps. Final missions of HW2 ended up very much as the first one did, your massive fleet obliterates the smaller but still powerful fleet with little challenge. Both times, something of an anti-climax.
Grim Fandango is I think the only other game which I just couldn't do - some of the problems involved solutions which would never come to me without some form of hints. Same goes for Riven. Although I got bored with that before I truly gave up, but I wasn't getting anywhere.

What the hell are you talking about? In Homeworld 2, you do NOT want a massive fleet. You'll get owned that way. In the second to last mission, I had my fleet maxxed out in everything when I arrived, so the Vaygr threw /eight/ cruisers at me simultaneously, and their support frigates and fighters. I got killed so badly.
That's when I learned that you should scuttle all frigates and corvettes before each mission. Those just don't pack enough value to be worth the enemies they generate, unlike destroyers or cruisers.

*shrug* I did just fine without artificially limiting any of my fleet, admittedly, my strike craft got obliterated to hell a few times and most of my fleet got decimated at the massive hyperspace gateway duking it out with the 5 or 6 enemy battlecruisers, but I just kept the flow of ships coming out and the last 2 missions were a breeze, although I wasn't completely maxed out as you were - I don't remember there being that many battlecruisers around. I lost my dreadnaught in combat with the enemy dreadnaught, and may have replaced a few ships, but steady production queues meant it was fine.
But that said, I can't remember what level I had the AI set to, or was that just in player vs CPU that you can change it?

That Level I beat Stricktly on Luck. All My troops were dead and I Had One building Left. (and it was being attacked) On repeated Playings I was able to have some troops retreat to the other side of the base or Hide somewhere on the map.

Regarding the HW2 scaling system, it should be noted that the patch altered it significantly. You could be talking about two different algorithms

I found the Doom clones on the Amiga, Alien Breed 3D 2 - The Killing Grounds being a telling example, to be generally quite unfair. I defy anyone to even get past the fourth level without some form of invulnerability + infinite ammo cheat. Otherwise, the Alien Breed series in general was pretty hard.

All the puzzles that WERE solveable were logical-all the information you needed was provided in the game, connecting the dots only required common sense and the willingness to think things through.

I never got around to beating Super Mario Bros. either. I beat Mario 3 and Mario World, but then I never beat the first, or even played the second.
There is some Star Trek game for the PS, where you fly a fighter. I found that game to be really hard. It didn't help that I wasn't interested enough to try to get good at it though.
