Well, BCI technology with EEG interface is the way to go.
But if the precision in pattern recognition isn't much higher than 96%, it's not going to be used in non-entertainment purposes (like controlling cars or wheelchairs) for quite a long time.
Though I guess eventually the precision will go up to 99,9999% and then technically you can use it anywhere in the industry.

If, for example, there were three different "tests" with an independent rate of error performed before each action, that ought to theoretically allow a final error rate as low as 0.0064% instead of 4%, e.g. 99.99% reliability. Albeit, that particular technique would have a disadvantage of having 1/3rd the speed of controlling the wheelchair. And that technique would depend on the chance of error in each different "test" being sufficiently independent events, so one could have the final error probability be 0.04^n theoretically, for n different types of "tests" performed before accepting an action. I'm not quite sure what you're getting at in terms of error, but the fact is that the digital signal which comes in from the interface can be compared within a given range. bottom-line though, is that everything depends on the human user to generate the appropriate signal. At this point, a wheelchair-type example would have to be customized to each individual.
