Phred or pyro or some mixture of the two. Oddly enough I rarely ever see anyone else use Phred since I figured it would be a simple variation on a common name.

I haven't used it in a long while, but in the past I used Lord Q'Daan
Poison Quill or similar was used for a lot of things.
In Day of Defeat I briefly went as Betty Paige (had recently learned how to make custom sprays).
In Natural Selection I was briefly Baby vs Onos.
I was Qutt in WoW, but I don't play that anymore.
Mostly these days I go by MissileDance.
In DotA I have to change name every few months because I end up on too many banlists thanks to either my network crapping out or my internet connection suddenly developing massive packet loss.
