I'll grab the area from central Europe down into the middle east. As for a history. The area in central Europe sufficiently devastated during the religious wars in Europe, Ended up being absorbed into what at one time was the nation of Turkey during the rebuilding efforts in the area. As the Polish Hegemony and the Iberian Federation were beginning to form the New nation of turkey took interest in it's neighbors activities. Rather then butting heads with the Polish Hegemony, Turkey decided to pursue interests towards the Middle east.
The Middle east had been gaining a significant Population influx as refugees from the Civil war in China. The nations found that they could not support their new population levels and the area remained in a third world state for quite awhile. When Turkey began Expanding into the middle east, it was able to Slowly unite the nations on the basis of providing food and other necessary resources required to move the Middle East out of it's hole. Eventually the Turkish Dominion was formed with minimal bloodshed (barring the occasional Religious Uprising).
The Dominion would have remained nothing compared to the other nations of earth, but near the end of the Dark Ages several worlds which held the earth as a sort of 'holy' or 'promised land'. It was a lucky stroke for the Dominion that they made contact with them, they'd managed to convince the outer worlds that the on the basis of older religions that they occupied the more holy parts of earth. The new Stellar Dominion is faced with a little problem currently. They have no National Access to a Space elevator, making life as a stellar power rather difficult.
As for the Star system I'd like to claim harvesting operations in the Oort cloud, as well as pluto and the area around Neptune as bases for access to Oort cloud operations.

Much of Catalonia was devastated by them, and the Basque lands were completely ruined. I never really elaborated, but I saw them as stemming from an increased polarization between the Catholic/European population and the rapidly growing Muslim/Immigrant population. Increasingly repressive anti-muslim laws, helped along by domestic terrorism, eventually lead to religious fanaticism and open fighting in European cities. The start of the Religious Wars as a continent-wrecking event came about when the Pope called for the German Crusade, after New Lutheran and Muslim groups took armed control of German cities in response to brutal registration laws and police violence. At that time, Poland's church schismed from the Catholic Church- the old church was now the Orthodox Church, based in Rome, and the new one was the Reformed Church, based in Krakow.
Other european nations were becoming more brutal, and repression was spreading to differnt minorities- which lead to warfare between states. By the end, nuclear weapons had been used on a number of cities, armies had split and mutinied many times, and the death-toll reached over a billion.

I've claimed North America, from Canada down to the Isthmus of Panama, plus the Caribbean and some polar bases in Greenland and Antarctica.
I think I can work my story into this.
The US basically ignored the rest of the world during those huge religious wars. When the AI got released, crashing the economy, the government helped out the open-source people, breaking up the monopoly that was Microsoft. Still with a weakened economy, the oil wars started, and Canada became a major energy source. Hippie environmentalists were ignored and the US started to change toward heavily nuclear power and electrical systems, but it wasn't enough. People didn't like this, and added to other internal conflicts, there was a second civil war. During this, some other countries, sensing the US's turmoil, decided to invade from Mexico. Well, a foreign invader was enough to get the US forces to ally and wipe them out. The US said "Blame Canada!" for their troubles, and invaded them too.
Basically, it's a "The American public got screwed over my big business and those dirty foreigners" and then America goes on a conquering rampage.
Oh, as for system holdings, I get Luna (since we sent people there to claim it first) and asteroid colonies. Maybe part of Mars, and Titan, if they're not already claimed.

Looking at this, I think my guys left during the Oil Wars when FTL was still highly experimental. In fact, reading MRDOD's OOB I got this image in my head.

I don't know if it's realistic that 40-something Billion Germans could come out of the much smaller number (perhaps half a billion) who left in 230 years, even if we do some cloning for manpower concerns during the early years, but, well, I did my best.

Hell, you could say that your mission was deleted from most mainfraims during all the server damage caused by Microsoft's death throes, and with the dawn of the Oil Wars the last of the hard documents saying that your guys ever even existed were lost as well. Might be a bit early, but we've got enough insane shit going on that I doubt it could not work.

The US weathered the Oil Wars, but emerged badly weakened. It continued to fade in power and prominence until the Religious wars, when the Old South bible-thumpers started another Civil War. In this commotion, many Americans left on colony ships to seek their fortunes elsewhere. Also, other countries, wanting a piece of the US, joined forces with Canada or Mexico to invade. The US Civil War was put on a hold as the foreign invaders were repelled, and the US forces kept pushing, through Canada and Mexico, eventually conquering all of Canada and all the way to the Panama Canal.
After about a century, the American colonists made contact again, prompting several states, who were full of hippies and experiencing a renewed protest against American Imperialism, to secede and join the Colonists, known as the Republic of America.
After some limited but fierce land and space battles, both sides agreed to a cease-fire, and there is an unspoken agreement to not fight on or around Earth, though there are large ground forces on both sides.
