An annoying problem: delayed sound

I have an annoying problem: sound doesn't always match action.

In videos, I hear actor's voices after they have closed their lips. In video games, the same.

I have changed my soundcard from a SounbBlsater 5.1 to a Audigity 7.1. Quality increased, still have the same problem. Any ideas? Should I post a DXdiag?

...I actually have this issue with a DVD player hooked to a TV, not even a computer thing. Movies start to 'slide' and they get de-synched. I dunno how that's even possible.
