I love the assumption that there even IS a plan. I don't run into many teams that actually have any kind of plan. The closest thing online public matches get to having a plan is having each player doing what he or she feels would best contribute to the team, and being intelligent enough to adjust their own personal goals to what the rest of the team is doing. That's still not the same thing as a plan; without a dedicated commander system, no player has any real chance of herding the feral, rabies infected cats that inhabit the interwebs into doing something in accordance with any strategy. Even with a dedicated commander, the chances of (1) that commander being even half-way competent and (2) the various 'rambos' actually listening are slim to none.

Simply discourage lone wolf behavouir, if you hate teamwork, then ensure that you just stick with single player or other game modes.
Give them a basic plan, in base assault, all it takes is one player to suggest which base to attack first, and tons of veteran players will agree with him, and attack that base first.
Ensure that being a rambo, and resulting in a loss will not win you any points. The top of the game lone wolfs will have no other choice but to have some sort of team work.
In some instances, when the team is losing the entire game, and all it requires is one single good rambo player to get his act together, and find a way to turn ensure the game can last longer and turn the tide. If a team lose a game, simply ensure that the losing team will not gain any personal scores.

Anyone ever faces the problem of resolving disagreement with team plans? I used to run into that problem back in DOTA allstars.
A valid disagreement between "should we rush the enemy because if we keep holding back, we will be defeated as their heroes scale up better with level and equipment or should we go defensive and concentrate on clearing lanes and then crippling heroes so that our heroes can scale up, as we're badly outclassed in anything outside of a 2 or 3v1 fight" can often result in an ongoing dispute that lasts up to the endgame itself.
The worst thing that could happen is some form of vacillation between two diametrically opposite plans that result in all the flaws and weaknesses and none of the benefits..... yet, I routinely see this happen in DOTA and WOW battlegrounds. Just interested to see how anyone here tries to resolve this issue.... especially since any plan would meet with some setbacks and would require continued, sustained investment for it to work.
