I would be inclined to conclude the students did cheat. The text book publisher's bank of questions is generally not available to students.

In my whole school/university life, I never, ever had one test with multiple choice questions. All you get is usually the question and a blank space to fill with your knowledge (or lack thereof). "Daring the gap" for the win 
And the professors I had usually involved their assistants in the corrections of the exams, which took up to one or two months, depending on the department.

How are students normally prevented from accessing it ?
Lets assume that a student has been cheating because they read something that they were specifically told not to. Naturally they deny it. How could someone prove that they had cheated ?
If you can't prove that someone used it and it is very helpful in passing the test, wouldn't banning it just be harming the honest students ?
