It happens when you have a warm layer (layer of air > 0C) aloft, and a thin layer of cold air (air < 0C) at the SFC. The warm air melts the snow into rain, and then the cold air chills the rain again, but it's not thick enough to result in rain freezing again before it the ground... so it hits the cold ground and instantly freezes then. You can also have the rain turn into supercooled water when passing through that layer, so it's liquid even though it's below the normal freezing temperature. As soon as it hits something - anything - wham! Instant freeze.
