What was so unsafe about Vader's TIE formation in the trench? They crashed, yes, but only when blindsided by the Millennium Falcon.

Wrong; the SSD has plenty of spare parts and maintenance crews.
Load of good thats going to do to the fighter stranded away from the SSD.
For those obviously routine occasions when a fighter has been prepped and maintained prior to its mission, sent out on an extremely short-duration hop to fire a missile and come home, and then broken down and stopped dead relative to the SSD during that short trip due to the simultaneous failure of both sublight and superluminal drive systems , they can just send out maintenance droids to fix it and bring it back home.

The real question here is who takes over the ssd once enough of the crew dies so that it becomes assailable?

Nobody. In that event, the ship would be scuttled.

Close formations tend to be accident-prone especially will little to no
manuvering space. The Tie escorting Vader would be much more usefull if
they were watching his back or waching for trouble.

Are you guys even familiar with the specs on these things?

1) A GAT-12 series is not a one man fighter. It is crewed by four.
2) Its three Ion cannons are capital ship scale weapons, don't act like they're fighter scale.
3) If its with Defenders and Missile Boats why are we using the older 12h instead of the 12j?
4) A 12j carried 12 proton torpedos and 18 concussion missiles a impressive amount of warheads, ne?
Their ion cannons give this craft a reasonable chance of disabling larger combat ships. The hull plating is so heavy that most starfighter lasers would have a tough time penetrating the armour, thereby making this ship impervious to all but the most powerful starfighter weapons.

These are very tough and powerful ships. They usually patrol entire sectors in pairs, take care of just about any problems, and come back.

These are the most deadly and capable fighters in the Empire. Wong estimates the Federation Capital ship force between 1000 and 2000 ships. Fighter use not being a common tactic among the Federation means there are few of those and I have yet to see captain launch shuttles or runabout to use them like fighters. That and without a lot of prior planning a big group would be about 39 vessels, like at Wolf 359. Against normal fighters yes you have a point but these aren't in any way normal.

These are the best of the best piloted by the best of the best, I'm sorry but with that much firepower between the Missile boats and Skiprays as well as the Defenders high performance the UFP fleet is in trouble. Assuming of course the Executor-class vessel doesn't just hack the UFP network with slicer droids take the codes we know exist from ST2, as well as any valuable information, and just shut their entire fleet down the same way Kirk brought the Reliant's shields down. Plus on a ship meant to service 12 squadrons keeping 3 squadrons in top nop shape is going to be a problem? Ammo concerns, considering how big that ship is, and its carrying missile boats, read ammo hogs, and so is equiped to service and keep them armed .

The ships that specialize in missiles, the ships that are chibi-capital ships like the UFP's Defiant in their realm, and the greatest Space superiority fighter in Star Wars Galaxy aren't suited for this? Attrition requires kills, which aren't in any way easy even for the Star Wars Galaxy against these craft.
