Wasn't there a Trek episode where we saw some insanely old Admiral get a magical de-aging medicine? As I recall, it was working great until he took a second dose.

Too Short A Season, with a drug called Cerebus Rejuvenator. It takes two years to have its full effect, is generally not exported from its homeworld, , painful and dangerous.

Not that high a price for immortality, of course, but it has a substantial fatality rate even for the species it was designed by.
