Wasn't there a Trek episode where we saw some insanely old Admiral get a magical de-aging medicine? As I recall, it was working great until he took a second dose.
Not that high a price for immortality, of course, but it has a substantial fatality rate even for the species it was designed by.

Very painful. Aliens are seldom
allowed to obtain the process,
but I managed it. I negotiated
a treaty for Cerberus II several
years ago, and they felt obligated
to honor my request for the
process.
