I've had my share of painful accidents, but the most embarrasing was when I was playing rugby at school. The ball had been dropped and whilst running I had to bend down to scoop it up, I missed it. I kept running for a few metres, having not mentally kept up with what was happening, just as I remembered to straighten back up I plowed right in to some guy.

When I was very young, I stood up from tying my shoe... and ran the bridge of my nose smack into the edge of the countertop. I don't personally remember it, but my mother says it spurted a nice tall fountain of blood for awhile. I still have the scar.
More embarrassing and painful was when I was touring a lightship in Astoria, OR. We were going down some particularly steep stairs. My parents offered to go in front of me so that if I fell down I wouldn't fall all the way down. I refused, going first. I got down first alright... on my ass. Again, when I was very young.

I have had a concussion and required actual stitches as a result of an injury sustained by a HOUSE CAT.
To be exact neighbor shot cat with water, cat ran straight up me (very sharp claws), jumped off me and onto the roof, where it dislodged a BBQ grill that someone had thrown on top of the room, BBQ grill hit me in the head knocking me unconcious....
