This has nothing on this one video our CJA class saw (pretty sure it was a DEA one, too).
In short, agent drives up, with gun in hand on the wheel. Manages to shoot their own agent because of this, and when the other guy gets out of the passenger door, the driver forgot to put it in park or whatever and almost ran his partner over. Then when all is said and done, and they cuffed the suspect, they did something else pretty stupid (don't remember if it was turning away from the suspect so noone was watching him, pointing guns carelessly at suspect, or both).
Again, I'm not 100% positive these were DEA agents (that video was from quite awhile ago) but I think it was.

Um ... no. Live rounds, to my knowledge, have never been allowed on film sets. Brandon Lee was killed when a blank discharged and propelled a bullet that was lodged in the barrel for some reason. There wasn't a live round in the clip.

The round that was loged in the barrel was a live round and it was there because they used the live round and the gun to film and earlier scene involving the gun having a bullet insertered directly into the chamber (which they obviously had to use a live round for to make it look right).
When the live round got logged in the chamber, the gun was taken to be fixed but some asshole threw it back into rotation.
