I think we see the result of a dual chain of command in ANH.
There is Vader as the second in the Sith order the Emperor's right hand man but to some extent outside the formal chain of command and not a part of the real Imperial chain of command.
Then there is Tarkin, a part of the Imperial goverment and the military command structure. Tarkin was probably one of Palpatine's cronies from way back, probably since the Clonewars. Tarkin knew both of Vader's Jedi past and of Obi Wan - he probably served with, or even commanded, them both during the wars.
By the time of TESB & ROTJ any last remnants of the old order have been swept away and Vader is given a chart blanche to win. Without Tarkin to isolate the Sith from the officers there are some clashes of personalities...

Tarkin had reign over Vader because the Death Star I was his personal pet project. Grand Moffs were more likely fuedal lords over their territories, answering to Palpatine, but seemingly otherwise independent. The regular Moffs were a lot more assimilated into the Imperial power structure and seemed like appointed governors; their territories seem more like administrative regions that how the Oversectors seemed like feudal fiefdoms.

These two facts, combined with the fact that the Death Star I was Tarkin's battlestation, and Vader was simply Palpatine's envoy to keep tabs on Tarkin's ambitions and plans, explains why Tarkin had pre-eminence. The Death Star I was completely Tarkin's turf.

In the ANH novelization Vader's thoughts are described, and he is humouring Tarkin's officially superior rank. He could easily kill him at any time if he wished, probably without censure from Palpatine.

In the early script of ROTJ, Jerjerrod is a Grand Moff analog to Tarkin who constantly picks at Vader and criticizes him. He is terminated in a corridor of the battlestation about mid way through the script.

Is it considered as truth that Veers died at Hoth, though? I think that would be stupid, personally. Having one of their best tactitians get killed because he was on the front line of a relatively small battle in the backwaters of the Galaxy seems to be a consistent problem for the Empire...

Veers more likely died on the Executor when it plunged into the Death Star. Veers was in charge of the Imperial Army units on that ship.

I'm pretty sure that Veers lived past the Endor incident. I think it's in one of the EU novels

Veers was wounded at Hoth.

The novelisation was retconned. Hobbie crashed his fighter into Veers' AT-AT. Both survived. Hobbie recovered. Veers lost his legs.

Maximilian Veers had an irrational dislike for cybernetics. He instead moved around on a repulsor chair; rather than recieve simple artificial prothesesis like Luke did or expensive cloned limbs.

He moved off the Executor with his forces.

Around this time, his son, Zevulon Veers, defected to the Alliance. He would later become a capital ship turbolaser station officer.

After Endor his unit ended up claimed by various warlords. They detested Vader and considered him his lackey, and thus took it out on him.

Veers' mental patterns were incorperated into many of Thrawn's Spaarti clones.

He eventually became part of the ground assault force for Military Executor Sedriss during Operation Shadow Hand. He led the ground assault on Balmorria's capital. His command vehicle was hit and destroyed. Veers died instantly.

Also, I believe he was disheartened by his son and continually slammed his Imperial Forces upon the enemy at Balmorra. So, eventually he was seen in the open and killed.
