Yes, he's always been in charge. What happened was that the inhabitants of heaven were exploring and found Earth along iwth its evolving intelligent species. However, in opening a portal back from earth, somebody made a mistake and opened one to the previously-unknown Hell. This they then seized, enslaving its original inhabitants (the orcs). Yahweh wanted to rule both Heaven and hell, Satan wanted one for himself and Satan's revolt was The Great Celestial War, a bitter fight that went on for a long time and ended more due to mutual exhaustion than anything else. But, Satan won that war and Hell became independent. Yahweh and Satan more or less drove other creatures from their dimension off Earth and shared it until Yahweh gave up on humans and gave the place to Satan.
2) How did Heaven/Hell first understand about their own afterlife and how to get there?
Well, they don't actually understand it; they believe it. They took the explanations from their leaders on faith. Risky thing to do that.
3) Did Yahweh know about how to "cross-over" before he created humanity? If so can we assume humanity was created for the sole and explicit purpose of providing energy for Heaven/Hell’s inhabitants crossover?
Yahweh didn't create humanity, he found earth where proto-humanity already existed. In fact, even earlier than that, demons/angels/humans all share a common ancestor who is native to Earth. What happened back then is, sadly lost in the mists of time. But, genetically, humans and demons are related and angels and demons are very closely related.
I'm not quite sure what you mean by cross-over here. If you mean between earth-dimension and heaven/hell dimension, then its happening all the time. If you mean from heaven/hell dimension to another dimension upwards, then its an unproven hypothesis that such can happen at all.
4) Has a crossover already been performed?
If you mean from heaven/hell upwards,, nobody knows. Beings in hell (and in heaven) get killed at regular intervals (especially when human weaponry gets to work) but nobody knows what happens. Demons (and angels) believe they get translated to a higher plane of existance using human energy to boost them but nobody knows.
5) If so by whom and was this before or after humanity's creation?
Lots of candidates but nobody knows. Remember, creatures in hell/Heaven
know as much about what happens to them after they die as we
know about what happens to us when we die. One of the major points of Armagesson is that believing something is entirely different from knowing something. There is no proof of any level of existance beyond Hell/Heaven all there is, is belief. For all we know, we may get one shot at life on Earth, a second shot in heaven/Hell and that's it.
6) If a crossover has already been performed what is the relationship between the hierarchical rank of the one crossing over and the ‘rank’ of the soul/s, time spent suffering, and intensity of suffering of the human soul/s used to provide the energy for the crossover?

7) How many (if any) human souls have been expended in crossovers to date?

Does the ‘greatness’ of a soul provide more energy for a heavenly crossover than a 'less great’ soul? If so does the opposite hold true for Hell's inhabitants, even though they themselves were once inhabitants of Heaven?
Again, most of this is unknown. The demons believe lots of things about the dead humans (one is that prolonging their 'second deaths' with as much agony as possible generates more energy from them and thus aids the demons in transitioning to their afterlife. But that's what they believe. Angels believe that a constant chorus of praise from their humans has the same effect.
Humans are beginning to turn science on those beliefs and that will cause some angst.
