Why did Voyager never adopt the gray uniforms?

For the first three seasons, its understandable, they have no contact with Starfleet. But they have more and more contact with them as the show goes on, and by the seventh season, they're even assigned missions.

Also, why did they not do it out of show as well?

It doesn't make sense I suppose to replicate themselves new uniforms when they're thousands of lightyears beyond known space. It's just a uniform, and it's not like anyone's going to really care that the poor ship of disaster didn't adopt a new spec.

Out of show it was the make the show look distinct from the movies and DS9.

In show, they didn't want to waste the replicator rations on new uniforms. Though this isn't a perfect theory since we know they can reduce items back to replicator energy. (Janeway tells Chakotay to this with her Birthday gift in the Year Of Hell episodes) So they could have chucked the old style uniforms into the replicator and got new style ones out.

That probably wouldn't be perfectly efficient though.

I think the "why should they" approach is the easiest explanation. Why should they bother with new uniform designs when the one they have is (to them) perfectly suitable.

And why should Starf;eet Command insist upon the change when they are more concerned with getting the ship home in the first place.

I think it also underscores that they're out there and not back home, when you see scenes of starfleet personnel back in the aq wearing the gray uniform in voyager. I see it as actually being effective in establisjing continuity

Why was that transporter duplicate Riker found living alone in a cave in the S6/S7 period of TNG still wearing his Starfleet uniform that was got phased out after the S1/S2 period of TNG?

It's also sort of like in some movies where a unit that's been cut off still has their old uniforms even after years of being isolated. They're worn, tattered, often roughly patched, but they still take pride in wearing them. I think the crew of Voyager, even if they had the ability to change uniforms, had worn the ones they had for so long and had gotten so far that they didn't want to give them up. It's a pride thing.

Nobody bothered to radio the specs for the new uniforms over?

Let's see... we need to let these people talk with their families, we need to radio any important information that Voyager needs to know such as maybe a possible problem with the Warp Core that needs to be checked, astrological data, any major issues we think they might need to know... hold on a tick... OMG THEY HAVE THE WRONG UNIFORM! DROP EVERYTHING AND GET THEM DRESSED PROPERLY!

The Federation is fucked! Truly and utterly fucked. Have you seen those Vulcan horoscopes recently?
